75+ Health Assessments

Patients aged 75 and over are eligible for a comprehensive health assessment with their GP under Medicare. This assessment is designed to identify any health concerns, review current medications, assess functional capacity, and discuss any support services that may be beneficial. If you or a family member is 75 or over and has not had a health assessment recently, speak with our reception team when booking to ensure enough time is allocated for this appointment.

Chronic Disease Management

Many older patients are managing one or more chronic health conditions. Our GPs work with seniors to develop structured care plans, monitor conditions over time, and coordinate care across multiple healthcare providers where needed.

A GP Management Plan and Team Care Arrangement may be developed for eligible patients, which can allow access to Medicare rebates for a set number of allied health consultations per calendar year. Your GP will discuss whether these plans are appropriate for your situation.

Medication Reviews

Managing multiple medications can be complex, and regular reviews are an important part of staying safe and well. Your GP can review your current medications, discuss any concerns, and work with you to ensure your medications are appropriate and well-managed.

Immunisations for Seniors

Older adults may be eligible for a range of vaccines, some of which are available at no cost under the National Immunisation Program. These may include the annual flu vaccine, the pneumococcal vaccine, and the shingles vaccine. Your GP can discuss which vaccinations may be recommended based on your age and health history.

Mental Health Support

Mental health is just as important in later life as at any other stage. Our GPs can provide mental health assessments, develop care plans, and connect older patients with appropriate support services. If you are concerned about your own mental health or that of an older family member, a conversation with a GP is a good starting point.

Allied Health Support

Older patients often benefit from coordinated care across a range of health professionals. At Lismore Doctors, our allied health team includes physiotherapy, podiatry, dietetics, psychology, and chiropractic services. Your GP can discuss which services may be appropriate and whether you are eligible for Medicare rebates through a Team Care Arrangement.

Support for Families and Carers

We understand that family members and carers play an important role in the health and wellbeing of older patients. We welcome family members or carers to attend appointments where the patient is comfortable with this, and our team is happy to answer questions and provide guidance to support the whole care network.

Bulk Billing for Seniors

All patients with a valid Medicare card are bulk billed for eligible consultations at Lismore Doctors. Pensioner Concession Card and DVA card holders are also eligible for bulk billing. Some services may incur a private fee. Our reception team will always advise you of any applicable costs before your appointment.
