Services Available for Aboriginal and Torres Strait Islander Patients

Aboriginal and Torres Strait Islander Health Assessment (715 Health Check)

Aboriginal and Torres Strait Islander people of all ages are eligible for a Medicare-funded comprehensive health assessment, commonly known as a 715 Health Check. This assessment is designed to identify health risks, review your current health, and connect you with the appropriate care and support services.

The assessment covers:

  • Physical health including blood pressure, weight, and cardiovascular risk
  • Chronic disease screening and management
  • Mental health and social and emotional wellbeing
  • Medications review
  • Immunisation status
  • Lifestyle factors
  • Referrals to specialists and allied health professionals where needed

If you have not had a 715 Health Check recently, we encourage you to book one. Please mention it when booking so we can allocate enough time for your appointment.

Children's Health Checks

Aboriginal and Torres Strait Islander children are eligible for health assessments that monitor their growth, development, and overall health. If you have concerns about your child’s health or development, a GP consultation is a good starting point.

Chronic Disease Management

Chronic conditions such as diabetes, cardiovascular disease, kidney disease, and respiratory conditions are areas of significant health concern for many Aboriginal and Torres Strait Islander Australians. Our GPs can work with you to develop a structured management plan, monitor your health over time, and coordinate care with other healthcare providers.

A GP Management Plan and Team Care Arrangement may be developed for eligible patients, which can provide access to Medicare rebates for allied health services. Your GP will discuss whether these plans are appropriate for your situation.

Closing the Gap — PBS Co-Payment Measure

Eligible Aboriginal and Torres Strait Islander patients may be able to access the Closing the Gap PBS Co-Payment measure, which reduces the cost of Pharmaceutical Benefits Scheme (PBS) medicines. Your GP can discuss whether you are eligible and assist you in accessing this measure.

Mental Health and Social and Emotional Wellbeing

We recognise that mental health and social and emotional wellbeing are deeply important aspects of health for Aboriginal and Torres Strait Islander communities. Our GPs can provide mental health assessments, develop care plans, and connect patients with appropriate support services and referrals.

If you or someone you care for is experiencing difficulties with mental health or wellbeing, we encourage you to speak with one of our GPs.

Immunisations

Aboriginal and Torres Strait Islander people may be eligible for additional vaccinations under the National Immunisation Program, including vaccines for pneumococcal disease and influenza. Your GP can review your immunisation history and discuss which vaccines may be recommended for you.
